Partners with Mom
Priority Partners is pleased to offer Partners with Mom, a pregnancy care management program for expectant moms. This program will help you manage your health during your pregnancy. The Partners with Mom health coaches work with mothers to help them recognize their own strengths and the ability to change unhealthy lifestyle behaviors during and after pregnancy. Moms have personalized monthly appointments with their coach to talk about nutrition, exercise, and other health behaviors. The program is available for up to 6 months after delivery and will help mothers stay healthy even after the baby is born.
