Juggling all your health care needs can be hard. If it’s too much to handle on your own, a care manager can help. Care managers are nurses and social workers who work with you and all your doctors to make sure you are getting all the care you need. They also teach you ways to make managing your health easier.

Care managers are nurses and social workers who work with you and all your doctors to make sure you are getting all the care you need. They also teach you ways to make managing your health easier.
Your care manager will:
- Find out what you need for your health care
- Make sure you get the services and supplies you need
- Help you get referrals if you need specialty care
- Help you get better at managing your conditions on your own
Work with a care manager if you are:
- Pregnant
- Ending a hospital stay
- Living with diabetes, COPD, hypertension, or another chronic health condition
Preventive Health
We’ll help you stay healthy and avoid complications. We will give you information you can trust about your health and reminders of services that you need. We may also set you up with a care manager who can help you understand a new diagnosis, work with all of your doctors, and guide you to get the most out of your health plan.
Transition of Care
When your health changes, we’re here for you. Whether you are leaving the hospital, have an injury, need new treatment, or something else, we can help you adjust to a change. We’ll support you with your specific needs and guide you back to your best health.
Complex Care
We can help you manage your chronic conditions. We work one-on-one with you to teach you how to take good care of yourself. We will also work with all of your doctors to ensure that you get any other care that you need.
Behavioral Health
Priority Partners’ licensed and specially trained behavioral health care managers can:
- Connect you with local support
- Help find appropriate providers
- Provide support and guidance
- Coordinate your care with your providers
- Provide resources to help understand and manage your prescription medications
