If managing all of your health care needs feels overwhelming, a care manager can help.

Care managers are nurses and social workers who can help you better understand and manage your health conditions. They are an advocate to make your care easier and your health better. You, your care manager and your health care providers can work together by:

  • Evaluating your wellness and your personal needs
  • Coordinating services and supplies for your health care
  • Building your skills so you can better manage your conditions
  • Helping with referrals to specialists

Work with a care manager if you:

  • Are pregnant
  • Are being discharged from a hospital stay
  • Have diabetes, COPD, hypertension or another chronic health condition

Preventive Health

Work with us to stay healthy. We will give you reminders about the health services you need and information you can trust.

Transition of Care

When your health changes — temporarily (an injury or inpatient stay) or long term (needing new treatment) — we will give you extra support.

Complex Care

If you have chronic conditions, we work with you one-on-one and collaborate with your providers to help you understand and take the best possible care of yourself.
