Take the small steps below to make a big change in your health. If you are due to have one or more of these services or tests, you could earn a benefits card.
A1c Control / Prenatal Visit / Postpartum Visit / Well Child Visit / Colorectal Cancer Screening
A1c Control
Benefits card amount: $50
What is this test? A blood test to check your blood sugar levels.
Who can earn this? Priority Partners members age 18 to 64 who have diabetes.
How can I earn this? Eligible members must have an A1C level at or below 9.0.
Prenatal Visit
Benefits card amount: $100
What is this visit? A checkup during the first trimester of your pregnancy. A doctor will check the health of you and your baby.
Who can earn this? Priority Partners members who are pregnant.
How can I earn this? Eligible members must have this visit during the first 12 weeks of their pregnancy.
Postpartum Visit
Benefits card amount: $100
What is this visit? A checkup to make sure you are healing well after having a baby.
Who can earn this? Priority Partners members who give birth.
How can I earn this? Eligible members must have this visit 7 to 84 days after delivery.
Well Child Visit
Benefits card amount: $75
What is this visit? A checkup to learn about your overall health or a well visit.
Who can earn this? Priority Partners members who are ages 12 to 21.
How can I earn this? Eligible members must have a checkup or a well visit with their doctor.
Colorectal Cancer Screening
Benefits card amount:
Colonoscopy: $100
Cologuard® At-Home Kit: $75
What is this screening? A colonoscopy allows your doctor to check your colon for anything unusual.
A Cologuard® test is an easy, at-home option your doctor can order. It checks your stool for anything unusual.
Who can earn this? Priority Partners members ages 45 to 85.
How do I get started? To earn your reward, complete one of the following:
- A colonoscopy, or
- A Cologuard® at-home screening test
If you qualify for Cologuard®, your doctor will order the kit and it will be mailed directly to your home.
