All Priority Partners members are receiving a new yearly health survey to complete. The short survey focuses on health-related social needs like food security, stable housing, employment and reliable transportation.
The survey takes only about 10 to 12 minutes to complete. Your answers will remain private and help us to better understand our members’ needs and create helpful programs.
