Here are three ways for Priority Partners members to obtain a blood glucose meter at no cost:

Which blood glucose meters are covered?
ADC’s Freestyle® and Precision Xtra® blood glucose meters are covered. These meters are available to Priority Partners members at no cost.
Members are limited to one meter per lifetime through this program. If a member needs an additional meter, their doctor may submit a prior authorization request to the Priority Partners Pharmacy department.
